Safe System of Work (PtW & LOTO)

This project involved updating and streamlining an existing permit to work and plant isolation system that was not being consistently applied, leading to a number of near misses associated with poor isolation. I reviewed the existing procedures and established that the overall process was too complicated and procedures too wordy for the permit issuers to apply the rules effectively. I rewrote the full suite of procedures aligning them with the HSE guidance on Permit to Work systems and plant isolations. I then developed a comprehensive training package delivered to the permit issuers. This resulted in a much greater level of compliance and consistency of approach as well as a reduced incidence of near misses associated with permits and isolations.
